Before Your Appointment

To save time, you can complete your paperwork prior to your appointment. Feel free to either bring your completed paperwork with you or you can scan and email it to us at Office@JustCatsClinic.com. If you are a new client, please fill out all three forms. If are a current client with a new addition, just fill out our new patient form. Make sure to fill out a new patient form for each cat.

Take a few minutes to think about your cat’s behavior lately- even if you are just coming in for an annual exam or vaccines. Now is the best time to have any questions you may have answered or any usual behaviors or symptoms notated in your cat’s records. Here’s some questions that might help get you started:

Has my cat been eating normally?
Drinking normal amounts?
Has my cat been making any unusual vocalizations? Vocalizing more than normal?
Has my cat been urinating normally?
Having normal bowel movements?
Has my cat been sleeping more than normal?

Day of Appointment

If you choose to fill your paperwork out upon arrival, please make sure to arrive 10 minutes prior to your scheduled appointment time. It is very helpful to have any prior records your cat may have, no matter how old they may be. We are happy to call your previous animal hospitals to obtain records.

For the safety of all patients and office cats, please make sure pets are kept either in a cat carrier or on a leash.
